To start from console(remember to activate "monkeytoy" beforehand): devmap zombie_theater
Definite classic!! My personal record is level 37 so far. I could easily reach higher levels if I would not get always bored of it latest at around 30. I guess you could probably play on it forever on with the right strategy. Generally, I reckon you should be using Teleporter 2 or more times in a round at higher levels.

"Five" - Pentagon

It takes place in the pentagon where you will need to raise the Def Con level from 1 to a safer level manually to be able to access the pack-a-punch machines and a good safe spot from where you can hold yourselves against the hordes of zombies. Great map for sure. Really hard to in Single Player though. Never made it past Round 10, myself. It also requires its own kind of strategy, I guess, for succession to higher rounds. If you are playing in Single Player mode you need to have a fluke to get some worthy weapons as soon as possible and you can then possibly make it past the second Lab Thief and then just go forward. It is an Arcade take on the whole zombies side games. Top-down camera view movement with the keyboard; aiming and shooting with the mouse, collecting scores and weapons etc this is how you make it through different levels with a "Prison break" at every tenth game.
